Postion Overview:
We are seeking a proactive and compassionate Support Coordinator to join our dynamic team. This role involves working closely with families to optimise their children’s NDIS plans and access necessary supports that enhance their child’s development and wellbeing.
The ideal candidate will:
have extensive experience and relevant qualifications in the social or behavioural sciences, disability, community or mental health and/or lived experience*
have an in-depth understanding of the NDIS Act, associated operational guidelines and other systems and frameworks
be passionate about the rights of children with disabilities and their families/carers
have experience with paediatric services or working with children with special needs.
be solution-focused, tenacious and creative in their thinking
be warm, caring, personable and engaging
have outstanding time management skills
have an up-to-date WWCC, or be willing to get one.
have minimum 1 year experience as a Support Coordinator, plus considerable relevant experience

What you’ll be doing:
• Collaborating with families to understand their unique needs and aspirations.
• Developing, implementing, and monitoring NDIS plans tailored to each family’s specific circumstances.
• Liaising with service providers and external partners to facilitate access to required services.
• Advocating for families’ needs within various systems and settings to ensure optimal care and support.
• Maintaining accurate records and reports to track progress and outcomes.
