Navigating your child’s supports can feel overwhelming, but our support coordinators are here to help. We’ll work with you to understand your child’s goals and make the most of their NDIS plan, offering unbiased recommendations on suitable services. You’ll always have full choice and control, with us by your side to support you. In many cases, the cost of hiring a support coordinator is covered by the NDIS, so feel free to reach out!
We are currently offering the following levels of Support Coordination:

This form of support is utilised to build your child’s ability to connect with informal, community and funded supports enabling them to get the most out of their plan and pursue their goals! We support you through:
Connecting you with the appropriate local service providers, such as therapists, support workers, and educational programmes.
Guiding you, as your child’s carer, through understanding and navigating the NDIS.
Coaching you on how to effectively utilise your child’s plan to achieve their goals.
Helping you resolve any issues that may arise with any current or potential service providers.

This form of support is intended to assist you to build the skills you need to understand and use your child’s plan. Our support coordinator will work with you to ensure variety of supports are used to increase your child’s capacity to maintain relationships and live more independently and inclusivly in their community. This includes:
Developing tailored support strategies that address specific challenges.
Establishing effective support networks with service providers.
Building your family’s capacity to manage crises and unexpected situations.
Providing regular updates and reports to the NDIS on your child’s progress.
Facilitating communication between therapists, support workers, and other professionals involved in your child’s care

Support coordination is an NDIS funded service designed to help participants (and their families/representatives) to coordinate the supports in their NDIS Plan to get the best out of their NDIS funding. Supports include informal, mainstream, community and funded supports.

Typically support coordination will be clearly listed on your NDIS plan. Support coordination will come under your child’s “Capacity Building” budget.

Our support coordinators utilise their in-depth knowledge of the NDIS to assist you with:
Determine their individual needs and development goals
Navigate the NDIS marketplace (including mainstream, community, informal and provider options) to find supports that are best suited to meeting their needs and goals
Communicate with support providers, to maximise the hours of support, and complete service agreements and bookings
Build capacity and exercise choice and control, for example by mentoring and teaching e.g. how to use the NDIS portal, monitor budgets and find providers
Monitor and record their progress towards achieving their goals
Prepare for an NDIS Plan Review and provide support during and after

A Support Coordinator's role is to help NDIS participants to understand their NDIS Plan, identify their needs and goals, monitor their progress and connect with the people and support that will enable them to live with choice and control.
A Plan Manager is responsible for the financial/administrative side of a participant's NDIS Plan. For example, the Plan Manager is responsible for organising payments, processing claims and invoices and tracking budgets. If you require plan management services, your support coordinator can help you to arrange this. (Currently we do not offer plan management)

Funding decisions are based on what is considered ‘reasonable and necessary for your child to pursue their goals, in addition to the support provided by family, friends, and other community and government services.
Where reasonable and necessary, support coordination will be included in the Capacity Building budget. This is a fixed amount for a support coordinator to help you use your plan.
If your plan describes (or ‘states’) the level of support coordination funded, you can only purchase this level (we offer levels 1 & 2) of support coordination.
If your plan does not describe the level of support coordination funded, you can choose to purchase the level of support coordination that suits your needs.

We will contact your current Support Coordinator to arrange the transition and ensure there are no disruptions to the delivery of your existing support services.
We will contact your service providers to let them know we are now your support coordinator contact.
